Pat Hobson works with health and social care organisations that are stuck, where the needs are complex, the stakes are high, and the usual answers no longer work. Her “person-led pathway” models for dementia, safeguarding and the management of complex cases are not just models, they are something she’s implemented on the ground, from hospital wards to care homes.
